on-sight team member health check

Step 1:

Take the temperature of your team member using the non-contact thermometer.

  • At any time if a team member does not feel well or when conducting the pre-shift wellness check they have a temperature over 100.4° please excuse them from work and send them home so they can seek medical attention, as needed.


Step 2:

Ask your team members the following questions:

  • Are you showing symptoms associated with COVID-19 (fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea)?
  • Have you been diagnosed with COVID-19?
  • Have you had close contact in the past 14 days with someone who has been diagnosed with COVID-19? Close contact is defined as being within 6 feet of an infected person for at least 15 minutes, providing care for someone with COVID-19, direct contact, been sneezed, coughed or somehow got respiratory droplets on you from someone with COVID-19.
  • Have you been told by a health care provider or public health official that you should self-quarantine due to potential COVID-19 exposure, or are you suspected of having COVID-19?


If the team member completes the temperature check and answered all questions correctly, it will be determined they are sage to work.


What to do if they have been in close contact with a COVID-19 positive person or have a temperature over 100.4°?


Request that they obtain a COVID-19 Test

  • To work, the team member must produce a negative COVD-19 test, quarantine for 7 days and continue to monitor symptoms
  • If the test is positive, the team member will need to follow all state and local guidance on returning to work
  • If not test is conducted, the team member will need to self-isolate for 10 days with no symptoms

If a positive case occurs, contact tracing must go back 7 days. All close contact team members informed. 


Team members who have tested positive for COVID-19 do not need to quarantine or get tested again for up to three months as long as they do not develop symptoms again.
